RCA Description
A delivery truck arrived at a busy distribution yard during the morning rush. The driver was instructed to back into Dock 6. A spotter was normally used for backing movements, but the assigned spotter had been pulled away to assist with another unloading task.

The driver began reversing using the mirrors and backup camera. At the same time, a warehouse worker walked behind the truck while carrying paperwork to another dock. The worker entered the truck's blind area and was struck at low speed by the rear corner of the vehicle.

The worker suffered a fractured wrist and bruising and was transported for medical treatment. The truck sustained minor damage.
Immediate Actions Taken
Provide medical attention to the injured worker. Secure the incident area and preserve relevant information. Notify site supervision and begin the incident investigation. Review backing procedures and reinforce the requirement for appropriate controls during reversing movements.
1) Common Contributing Factors
1. Physical Environment
Factor Factors Notes
The distribution yard was busy with multiple vehicles and personnel present. Yes / No
There was significant traffic in the vicinity during the morning rush. Yes / No
Dock 6 had limited visibility due to nearby structures and vehicle positioning. Yes / No

A Structured Investigation

One incident. Multiple layers to investigate.

SafeSHIFT helps organize the investigation so the analysis goes beyond the immediate event and examines what allowed it to happen.

01

Describe the Incident

Capture what happened, where it happened and the immediate circumstances surrounding the event.

02

Review Contributing Factors

Examine conditions across the workplace rather than focusing on a single action or individual.

03

Investigate Further

Use incident-specific questions to identify what still needs to be confirmed or understood.

04

Identify Possible Root Causes

Organize the potential underlying causes revealed through the investigation.

05

Define Corrective Actions

Turn findings into specific corrective and preventive actions with ownership and status.

06

Track What Changes

Identify metrics and communication strategies that help verify the response is actually working.

Look Beyond the Obvious

Six areas of the incident worth examining.

A meaningful RCA considers the environment around the event — not just the final action that immediately preceded it.

01

Physical Environment

Site conditions, traffic, visibility, layout, weather and other environmental factors.

02

Training & Capability

Whether workers had the knowledge, preparation and capability required for the task.

03

Human Behavior

Actions, situational awareness, distractions and other human factors surrounding the event.

04

Systems & Processes

Procedures, staffing, operational controls and the systems that influence how work gets done.

05

Equipment & Tools

Equipment condition, safeguards, warning systems and tools available at the time.

06

Communication

How expectations, hazards, changes and responsibilities were communicated before and during the work.

Go Deeper

The immediate cause isn't always the root cause.

A worker walking behind a reversing truck explains what happened. It doesn't necessarily explain why the conditions existed for it to happen.

What happened
A worker entered the blind area of a reversing delivery truck.
Immediate factor
The truck was reversing without the normally assigned spotter.
What the RCA investigates
Why was the spotter unavailable? Why could the maneuver continue without one? What controls, staffing, communication or yard-design conditions allowed the exposure to exist?
The Finished RCA

From incident details to an actionable investigation.

Contributing Factors

Structured factor tables across the major areas that may have influenced the incident.

Investigation Questions

Questions tied to the event that help the investigator determine what still needs to be verified.

Possible Root Causes

Potential underlying causes organized for investigation, confirmation and documentation.

Corrective Actions

Recommended corrective and preventive actions with space for owners, due dates and status.

Metrics to Track

Measures that can help determine whether controls and corrective actions are improving performance.

Communication Strategy

Practical ways to reinforce lessons learned and communicate changes to the workforce.

Incident-Specific Questions

Know what to investigate next.

SafeSHIFT turns the incident details into practical questions that help guide the investigation.

What specific protocols were in place for spotters?
Were all personnel aware of the truck's movements?
How often is training conducted on reversing procedures?
What visibility aids were unavailable in the yard?
Was the spotter's absence communicated?
What measures could prevent a similar incident?
Turn Findings Into Action

The analysis shouldn't end with a cause.

SafeSHIFT carries the RCA into corrective and preventive actions so the investigation can lead to meaningful change.
